2007 Conference Survey Form
We need your help to improve our future State Conferences. Please fill out the survey form and attach in your email to Chiu-Ling Lin, 2008 State Conference Coordinator. Your input is very much appreciated.
Registration Forms
All the 2008 registration forms allow users to use Adobe Acrobat 8 Professional or Acrobat Reader version 8.0 or higher to type in and save directly into the computer. If you do not have the newer version installed in your computer, the forms will not work properly for you. Please attach the saved form(s) to the corresponding event coordinator(s) through email first. However, the registration process will not be complete until the check is sent and postmarked by the deadline. Please also include a printout of your registration form along with your check. This will greatly reduce the human errors for all the events. We do not have the electronic payment system set up at this moment. Thank you very much for your cooperation.
